Four patients die after one doctor performed the same operation on three of them

It has recently come to light that four patient deaths following heart valve operations were only investigated at the departmental level. It was concluded that medical malpractice did not result in the deaths. Three of the four deaths involved the same operation conducted by the same surgeon. He has since ceased doing this type of operation but it is believed that investigations should have been conducted at a higher level.
