New mother dies from midwife's error

A new mother who died within hours of giving birth due to septicemia was earlier reviewed by a midwife who was told that the mother was unwell with fevers and noticed a red mark on her abdomen. The midwife had forgotten her thermometer when she conducted the home visit and failed to notify a doctor of any concerns regarding the new mother's condition. The woman developed septic shock and was sent to hospital where a hysterectomy was performed. She died during surgery due to a heart attack secondary to the effects of septicemia.
