  • Physical examination
    • Determine if swelling is pitting or non-pitting i.e. compress the swollen area with the thumb for about 15 seconds. If the skin is indented and slowly refills the swelling is called "pitting". Bilateral foot pitting may suggest varicose veins, congestive cardiac failure, cirrhosis of the liver, nephrotic syndrome, Beri- Beri, malabsorption syndromes. Unilateral foot pitting may suggest deep venous thrombosis or compression of the large veins by a tumour or lymph nodes. Non-pitting swelling may suggest hypothyroidism or obstruction of the lymphatics
    • Cardiorespiratory examination to detect any evidence of congestive cardiac failure
  • Blood tests
    • Full blood count, ESR and blood cultures, if suspect cellulitis or osteomyelitis
    • D-dimer is an indicator of deep venous thrombosis
    • Thyroid function tests
    • Renal function tests
    • Serum albumin level
  • Urine tests
    • Urinalysis to detect proteinuria
  • Radiological investigations
    • Duplex ultrasound scan, impedence plethysmography and contrast venography are useful to detect deep venous thrombosis
    • X-Ray and CT Scan of involved swollen area may be useful if suspect bone pathology
    • Bone scans are often useful in diagnosing osteomyelitis and obscure fractures
    • Lymphangiography will be helpful in the diagnosis of lymphatic obstruction
    • CT scan of the abdomen or pelvis may demonstrate malignant out lymph nodes that may be compressing large veins
