Treatment of postherpetic neuralgia begins with prevention of shingles, which can lead to the condition. The best protection from shingles is receiving the shingles vaccination (varicella zoster virus vaccine). The vaccine is generally given to adults over the age of 60 who have had chickenpox.

There is no cure for shingles or postherpetic neuralgia. However, antiviral medications are often prescribed to reduce the severity and duration of the shingles and reduce the risk of developing postherpetic neuralgia. These medications include acyclovir, valcyclovir, or famcyclovir. Corticosteroids may also be prescribed to reduce the risk of developing postherpetic neuralgia.

The pain of postherpetic neuralgia is very difficult to treat and may require the use of a combination of medications. Medications that may be prescribed include an antidepressant drug, such as amitriptyline or nortriptyline. Anticonvulsant medication, such as Dilantin or Tegretol may offer some relief of pain. Topical medications that include the drug lidocaine may be prescribed. Capsaicin, which contains hot chili peppers, may also be recommended.
